What the library publishes

Clinical Documentation Library publishes free documentation templates, filled-in examples and writing guides for psychologists, therapists, counselors, social workers and other mental health professionals. They cover progress notes, treatment plans, assessment reports, clinical letters, insurance and billing documents, practice forms, supervision and forensic reports, along with state-by-state summaries of informed consent and telehealth rules and an ICD-10 code lookup. Every template is free to read, copy and adapt for your own practice.

Pages are educational, not clinical, legal or billing advice, and they do not replace your own professional judgment, your payer's requirements or your licensing board's rules. See the disclaimer.

What pages must not contain

  • DSM diagnostic criteria. A page may name a diagnosis and its ICD-10-CM code and describe it in plain language, but must not reproduce the DSM's criteria.
  • Test items. A page may name an assessment instrument and give its score range or published cutoffs, but must not reproduce its questions, items or rating scales.
  • Official CPT descriptors. CPT code descriptions must be original plain-language summaries, not the American Medical Association's descriptor text.

A page that reproduces any of these breaks this policy. Please report it.

How sources are cited

Every template and guide page must have an External Resources section linking to related professional guidelines, government and payer references, and tools. State pages and the state-by-state tables name the governing statute where there is one, and each state page links to that state's licensing board. Wording taken from a source must go in quotation marks, with the source named.

What the "Updated" date means

The "Updated" or "Last updated" date on a page is the date the page's own text or data last changed, taken from the site's version history. It is not a review date: it does not mean the information was checked against current rules on that day. Laws, payer policies and code sets change, so confirm requirements with the payer, licensing board or regulator before you rely on them.

How to report an error

If something on a page is wrong, out of date, or reproduces material this policy says it must not, email contact@myclinicalwriter.ai with the page's address and what needs fixing. A link to a source helps. When a correction changes a page, its Updated date changes too.

Who runs and funds the site

Clinical Documentation Library is built and maintained by the team behind My Clinical Writer, a documentation tool for mental health professionals. Some pages link to My Clinical Writer, and those links are how the site is funded. Some pages also show promotions for Digital Front Desk, another product from the same team. The About page describes the relationship with My Clinical Writer.
